Healthier Together Oregon is our 2020- 2024 State Health Improvement Plan. It names strategies to address different social issues that affect health.
These include:

- Health care and mental health treatment access
We are just getting started in this effort. We know that many regional and community-based organizations are already doing awesome work to support their local communities.
The Oregon Health Authority (OHA) would like to connect with your organization. The purpose is to find shared goals. It is also to connect you with other Healthier Together Oregon partners.

Partner Spotlight
Airway Science for Kids – STEM education for priority populations
Airway Science for Kids (ASK) works to address inequities across Oregon communities by providing accessible informal STEAM (science, technology engineering, technology, art and math) education. They do this by inviting fifth grade students in north Portland elementary schools to learn what it takes to design and fly an airplane, by prompting high school students across the Portland metro area to envision themselves in a thrilling and attainable career in space exploration, and so much more. In addition to encouraging these students to see themselves in the cockpit of a diverse future workforce, ASK supports their families as well; by offering meal programs, wraparound social safety net resources, and a safe place to land at their Aerospace Training Center. They are dedicated to inspiring healthier families and healthier communities.
Community Volunteer Network – Reducing loneliness and increasing social connectivity of older adults in Southern Oregon
Community Volunteer Network provides meaningful volunteer opportunities and direct services for seniors. Older adults who engage in volunteerism report fewer symptoms of depression, fewer feelings of isolation and improved overall health. Their volunteers support the health of older adults in our community through the following programs:
- Age Wise Age Well volunteers provide emotional and social support through peer counseling. These volunteers aid older adults in navigating through challenges associated with aging.
- Senior Health Insurance Benefits Assistance (SHIBA) volunteers help Medicare beneficiaries understand and make informed decisions about their health insurance choices.
- Call-A-Ride volunteers provide free non-emergency medical transportation to the elderly.
- Foster Grandparents support youth by serving as a role model and mentor to children in local schools and nonprofit programs.
Creating Opportunities – Programs supporting Oregonians with disabilities
Creating Opportunities is a community based organization based in Salem serving Marion, Polk, and Yamhill counties. Their mission is supporting children and adults with disabilities, their families, and communities in “creating opportunities” together. They believe in:
- Empowering families through education about disabilities, resources & service delivery systems.
- Connecting families to build real and meaningful relationships.
- Celebrating the strengths, unique gifts, and personal achievements of individuals with developmental disabilities.
Creating Opportunities encourages families to maintain a positive outlook while acquiring knowledge and taking action to create and expand resources in their local community. They also promote active participation of individuals who have a developmental disability in all facets of life at home, at work and in the community.
Perinatal Care Continuum Central Oregon – Cross sector partnerships for improved birth outcomes
Healthy birth outcomes are linked to being able to access engaged and validating prenatal health care early in the pregnancy. This is very important for African American and American Indian/Alaska Native parents. This is because these parents experience greater health risks during pregnancy due to the physical toll of racism and poorer birth outcomes.
The Perinatal Care Continuum of Central Oregon is a model of partnerships across different sectors. Their goal is to improve the health of parents and babies.
The program helps pregnant parents:
- Get enrolled in the Oregon Health Plan and find a provider
- Sign up for WIC for food support
- Access transportation to appointments
- Get help with other support services
Northwest Portland Area Indian Health Board (NPAIHB) – Dental health aide therapists as an alternative healthcare delivery model
More than one million people in Oregon live in areas with a shortage of dentists. Since 2016, NPAIHB has been working on a pilot to expand access to dental care. This pilot creates a new kind of provider that has been trained as a dental hygienist. But they can also do some of the procedures that dentists perform. Healthier Together Oregon plans to expand this model to increase access to dental health care in Oregon.
